You will need to send us:

• A completed and signed application form
  
• Two reference forms.

• A completed and signed medical form.

• Administration fee of £25GBP or $50USD. Please make cheques payable to: The Factory.
  
  We cannot process applications until we have received all of the above and we require the originals of all these documents. However if you are applying after June 1st you can fax the documents, as well as sending the originals, to speed up the process.
